M&D Employment Forms

All forms need to be completed in blue or black ink, and by submission you certify that information contained in each form is true and complete. Any false information may be grounds for not hiring or for immediate termination of employment at any point.

Employee Application Form

Emergency Contact Form — The emergency contact form provides information regarding the individuals that should be contacted in the event a traumatic event occurs on a Cypress Solutions LLC, job.

Employee Data Form

I-9 — You are required under the Immigration and Reform Control Act to use Form I-9, Employment Eligibility Verification to confirm your eligibility to work in the United States.

I9 form

W-4 — The Internal Revenue Service (IRS) requires every employee to complete the Form W-4. The form includes information on marital status, number of dependents and additional withholding amounts.

L4 — The state of Louisiana requires every employee to complete the Form L-4 so that your employer can withhold the correct amount of state income tax from your hourly pay / salary.

Drugs, Alcohol, and Other Prohibited Behaviors Drug Free Workplace Policy — It is the policy of Cypress Solutions, LLC to hire only persons free from any evidence of illegal use of controlled substances or other drugs including alcohol.

Certification of Vaccination Form — The purpose of this form is to take steps to prevent the spread of COVID-19, to protect the health and safety of all Federal employees, onsite contractors, visitors to Federal buildings or Federally controlled indoor workspaces, and other individuals interacting with the Federal
workforce.

Criminal Background Form — This form will be submitted during pre-employment as a legal criminal history check. It is the responsibility of the applicant to read and understand the instructions (click to view document) to complete a criminal background check with the Louisiana Department of Public Safety and the Federal Bureau of Investigation (FBI) located at 7919 Independence Boulevard Baton Rouge, LA 70806.

General Disclosure Form

Bureau of Criminal Identification and Information Authorization form is used in conjunction with the General Disclosure Form (above) when requesting background checks.
